Akte 128 jaar 1906 Tekst Ten jare duizend negen honderd en zes, den zes en twintig November te twee uren namiddag, verschenen voor ons Edmondus De Poorter, schepen, afgevaardigde Ambtenaar van den Burgerlijken Stand der gemeente Bornhem, arrondissement Mechelen, provincie Antwerpen, Joannes Pontzeele, fabriekwerker, oud zeven en dertig jaren en Joannes Verdickt, brievendrager, oud veertig jaren, beiden woonachtig in deze gemeente, de eerstgenoemde vader van den overledene en de tweede gebuur, welke ons vertoond hebben een levenloos kind van het mannelijk geslacht, waarvan de geboorte niet ingeschreven is, en welk zij verklaren dat gisteren om negen ure des avonds uit den schoot gekomen is van Albertina Steensens, herbergierster, oud drij en dertig jaren, woonachtig in deze gemeente, echtgenoote van den eersten verklaarder Joannes Pontzeele. Wij hebben ons van voormeld overlijden verzekerd. Waarvan akte, in dubbel, door ons ten Gemeentehuize opgemaakt, en na voorlezing geteekend door ons met de verklaarders.
