    Constantinus Steensens, daglooner, oud zes en wtintig jaren, geboren & woonachtig in deze gemeente, welken ons vertoond heeft een kind van het vrouwelijk geslagt, op ,heden om drij uren 's morgens geboren in deze gemeente, van hem verklaerder & van zijne egtgenote Dorothea van den Bossche, oud een en twintig jaren, alhier geboren & aen welk kind hij verklaerd heeft te willen geven dn voornaem van Josephina.
    De voorneomde vertooning & verklaring gedaen in de tegenwoordigheyd van Ferdinandus Heiremans, kleermaker, oud zeven en veertig jaren en Carolus Verleysen, schrijver, oud zes en zestig jaren, beyde alhier woonende.
    En hebbende de getuygen dezen akt benevens ons geteekend, den vader heeft verklaerd niet te konnen schrijven bij faute van geleerdheid, na gedane voorlezing.

    Constantinus Steensens, oud zeven en twintig jaren en Benedictus Michiels, oud negen en vijftig jaren, beyde daglooners te Bornhem, den eerstgenoemden vade der overledene en den tweeden gebuer, welke ons hebben verklaerd dat op heden om twelf en half uren 's morgens Josephina Steensens, oud acht maenden, geboren & woonachtig in deze gemeente, dochter van den eersten comparant Constantinus Steensens & van Dorothea van den bossche, beyde in leven, in hare ouders wooning alhier overleden is.
