It is my hypothesis the Family Search collaborators took the Nov 1975 death date without the day of the month from the social security death index. The SSDI provides the last SS address of record. It does NOT provide the PLACE of death. We might GUESS she died at her residence, BUT, she may have been taken to a hospital in a different town where she died OR she might have died whilst visiting a relative in another location. - D.A.
