    Leopoldus Carolus De Keersmaecker, landbouwer, oud twee en dertig jaren, geboren en woonachtig te Bornhem, welken ons vertoond heeft een kind van het mannelijk geslacht, op heden om twee uren 's nachts, geboren in deze gemeente, van hem verklaerder en van zijne echtgenote Maria Juliana Verheyden, oud twee en twintig jaren, hier geboren en aen welk kind hij verklaerd heeft te willen geven den voornamen van Cornelius Carolus.
    De voormelde vertooning en verklaring gedaan ter tegenwoordigheid van Joannes Baptista De Kuyper, veldwachter, oud vijf en vijftig jaren en Franciscus Gabriels, herbergier, oud acht en veertig jaren, beide woonachtig in deze gemeente.
    Waarvan akte, door ons ten Gemeentehuize in dubbel opgemaakt, en na voorlezing geteekend met den vader en de getuigen.

    Leopoldus Carolus De keersmaecker, oud vier en zestig jaren en Amedeus De keersmaeker oud zeven en veertig jaren, beiden landbouwers, woonachtig in deze gemeente, de eerstgenoemde vader der overledene en de tweeden kozijn, welke ons hebben verklaard dat heden om één uur en half des nachts, in zijne ouders woning alhier, overleden is Cornelius Carolus De keersmaecker, zonder beroep, geboren te Weert, den twee en twintigsten januari achttien honderd acht en zestig en woonachtig te Bornhem, zoon van den eersten komparant Leopoldus Carolus De keersmaecker en van Maria Juliana Verheyden, landbouwster, ook woonachtig te Bornhem, weduwenaar van Joanna Juliana De waet, alhier overleden.
    Wij hebben ons van voormeld overlijden verzekerd. Waarvan akte, door ons ten Gemeentehuize in dubbel opgemaakt, en na voorlezing geteekend met de komparanten.
