    Ten jare duizend negen honderd en acht, den een en twintigsten April te negen uren voormiddag, verschenen voor ons Edmond De Poorter Schepen afgevaardigde Ambtenaar van den Burgerlijken Stand der gemeente Bornhem, arrondissement Mechelen, provincie Antwerpen,
    Petrus Piessens oud vijf en veertig jaren en Franciscus Van Lent oud drij en veertig jaren, beiden daglooners woonachtig in deze gemeente, de eerstegenoemde schoonzoon van den overledene en de tweede gebuur, welke ons hebben verklaard dat gisteren om elf ure voormiddag in zijne woning, Kloosterstraat, alhier overleden is Carolus Laevaert dagloner geboren te Bornhem den negen en twintigsten meert achttien honderd een en veertig en daar woonachtig, zoon van Florentinus Franciscus Laevaert en van Joanna Catharina Vertongen, beiden overleden te Bornhem, echtgenoot van Petronilla Willaert huishoudster oud drij en zestig jaren woonachtig in deze gemeente.
    Wij hebben ons van voormeld overlijden verzekerd. Waarvan akte, in dubbel, door ons ten Gemeentehuize opgemaakt, en na voorlezing geteekend door ons en de twee voornoenmde verklaarders.
