    Carolus Laevaert daglooner, oud veertig jaren geboren en woonachtig te Bornhem, welken ons vertoond heeft een kind van het vrouwelijk geslacht heden om vier uren 's morgens geboren in deze gemeente van hem verklaarder en van zijne echtgenote Petronella Willaert oud zeven en dertig jaren hier geboren en aan welk kind hij verklaard heeft te willen geven de voornaam van Carolina.
    De voormelde vertooning en verklaring gedaan ter tegenwoordigheid van Julianus Muys winkelier oud acht en veertig jaren en Ludovicus Merckx mandemaker oud twee en dertig jaren beide woonachtig in deze gemeente.
    Waarvan akte ,door ons ten Gemeentehuize in dubbel opgemaakt, en na voorlezing geteekend met de getuigen,de vader heeft verklaard niet te kunnen schrijven bij gebrek van geleerdheid
