    Franciscus Verheyden, landbouwer oud vijftig jaren geboren en woonagtig te Bornhem, welken ons vertoond heeft een kind ven het mannelijk geslagt op heden om vijf uren 's morgends geboren in deze gemeente van hem verklaerder & van zijne egtgenote Maria Fillagia Hermans oud een en twintig jaren hier geboren & aen welk kind hij verklaerd heeft te willen geven den voornaem van Carolus.
    De voornoemde vertooning & verklaring gedaan ter tegenwoordigheid van Carolus Verleyden schrijver oud vijf en zestig jaren en Petrus Joannes Van Gucht schilder oud zes en twintig jaren beide woonachtig te Bornhem.
    En hebben de getuigen dezen akt benevens ons getekend, den vader heeft verklaerd niet te kunnen schrijven bij faute van geleerdheid, na gedane voorlezing.
