    Ten jare duizend acht honderd negen en negentig, den vijftienden december te negen ure voormiddag, verschenen voor ons Désiré Van den Abbeele, Schepen, gedelegeerden Ambtenaar van den Burgerlijken Stand der Gemeente Bornhem, provincie Antwerpen,
    Henricus Goossens, oud negen en vijftig jaren en Alphonsius Goossens, oud een en dertig jaren, beiden landbouwers, woonachtig in deze gemeente, de eerstgenoemde broeder der overledene en de tweede kozijn, welke ons hebben verklaard dat gisteren om tien uren en half des avonds in hare woning alhier overleden is Joanna Catharina Goossens, landbouwster, geboren te Bornhem den zeven en twintigsten april achttien honderd acht en dertig en daar woonachtig, ongehuwde dochter van Michael Goossens en van Francisca De Schryver. beiden overleden in deze gemeente.
    Wij hebben ons van voormeld overlijden verzekerd. Waarvan akte, in dubbel, door ons ten gemeentehuize opgemaakt, en na voorlezing geteekend met de komparanten,
