DE COMMISSARIS DES KONINGS IN DE PROVINCIE GELDERLAND verklaart, dat Hermanus Berendsen, geboren te Rheden, den 8 October 1859, wonende te Velp, van beroep metselaar, zoon van Hermanus en van Henderikka Wilhelmina Köhler, beiden wonende te Velp, in het inschrijvings-register van de gemeente Rheden van het jaar 1878, voor de lichting van het jaar 1879, is ingeschreven; dat hem bij de loting is ten deel gevallen No. 46; en dat hij vervolgens door den Militieraad uithoofde van een broeder in dienst, van den dienst is vrijgesteld.
Gegeven te ARNHEM, den 26 Juni 1886.
De Commissaris des Konings in de provincie Gelderland,
