Het formulier vermeldt: "Hij (= de ambtenaar) verklaart daarenboven, dat deze persoon behoeftig is en geene genoegzame middelen bezit om de kosten van zegel-, registratie-, griffie en afschriftrechten te betalen; dat bijgevolg die persoon al de noodige stukken om huwelijk aan te gaan kosteloos mag bekomen."
