Dewelke ons verklaarde, dat op den 26 ste dag der maand Junij dezes jaars des namiddags ten 4 ure, binnen deze Gemeente in het huis Wijk B-No 502 is geboren een kind van het vrouwelijk geslacht uit hem de declaran en zijne huisvrouw Trijntje Blom, zonder beroep, wonende alhier aan welk kind gegeven wordt de voornaam van Teuntje.
De getuigen waren: Hendrik Slotboom, 56 jaar, arbeider en Teunis van de Pol, 28 jaar, Sajetwa.....beiden wonende te Veenendaal.
(De vader verklaarde dat de getuigen niet konden schrijven, als hebbende zulks niet geleerd en niet tekenden)
