      Heden den zeventienden Januari achttien honderd een en tachtig, compareerde voor ons, Philippus Verhagen Metman, Burgemeester, Ambtenaar van den Burgerlijken Stand der gemeente Rijswijk, Provincie Zuidholland, Frederik van Valkenburg, oud vijfentwintig jaren [1856], arts, wonende alhier. Dewelke ons heeft verklaard, dat op den vijftienden dezer maand, des middags ten twaalf ure, in het huis, staande binnen deze Gemeente, gemerkt wijk A, numero honderd vijf en tachtig is geboren een kind van het mannelijk geslacht, van Petronella Johanna van Biezen, ongehuwd, dienstbode, wonende te 's Gravenhage, aan hetwelk hij verklaarde de voornaam te geven van Cornelis.

      Deze verklaring is geschied in tegenwoordigheid van Hidde de Vries, oud achtenvijftig jaren [1822], veldwachter, wonende alhier, en Cornelis Intveen, oud negenenveertig jaren [1832], veldwachter, wonende alhier.

      Wij hebben hiervan deze acte opgemaakt en voorlezing aan de comparant en de getuigen hebben zij deze nevens ons geteekend.
