    Ten jare duizend negen honderd en één, den zes en twintigsten februari ten negen ure voormiddag, verschenen voor ons Edmond De Poorter, schepen afgevaardigde Ambtenaar van den Burgerlijken Stand der gemeente Bornhem, provincie Antwerpen,
    Josephus Yon, brouwersgast, oud veertig jaren en Josephus De Schryver, daglooner, oud twee en dertig jaren beiden woonachtig in deze gemeente, de eerstgenoemde vader der overledene en den tweeden gebuur, welken ons hebben vertoond een levenloos kind van het vrouwelijk geslacht, wiens geboorte niet ingeschreven is, en welk zij ons hebben verklaard dat gisteren om elf uren en half des nachts uit den schoot gekomen is van Coleta Ludovica Eggermont, huishoudster, oud veertig jaren, ook woonachtig in deze gemeente, echtgenoote van den eersten komparant Josephus Yon.
    Wij hebben ons van voormeld overlijden verzekerd. Waarvan akte, door ons ten Gemeentehuize in dubbel opgemaakt, en na voorlezing geteekend met de komparanten.
