    Carolus Ludovicus De Schryver, daglooner, oud vijftig jaren geboren en woonachtig te Bornhem, welken ons vertoond heeft een kind van het mannelijk geslacht, gisteren om vijf uren des avonds, geboren in deze gemeente, van hem verklaerdern en van zijne echtgenote Carolina Ivens, oud negen en twintig jaren, hier geboren en aen welk kind hij verklaerd heeft te willen geven de voornamen van Jacobus Alphonsius.
    De voormelde vertooning en verklaring gedaan ter tegenwoordigheid van Joannes Franciscus Segers, oud drij en veertig jaren en Joannes Gregorius Daman, oud een en dertig jaren, beide veldwachters, woomachtig in deze gemeente.
    Waarvan akte, door ons ten Gemeentehuize in dubbel opgemaakt, en na voorlezing geteekend met den vader en de getuigen.

    Carolus Ludovicus De schryver, oud vijftig jaren en negen, zeg Jacobus Ivens, oud negen en veertig jaren, beide daglooners woonachtig in deze gemeente, de eerstgenoemde vader van den overleden en den tweeden oom, welken ons hebben verklaerd dat gisteren om negen uren en half des avonds in zijne ouders woning alhier overleden is Jacobus Alphonsius De schryver, geboren te Bornhem den vierden juli achttien honderd vijf en zevenetig en daer woonachtig, zoon van den eersten Komparant Carolus Ludovicus De Schryver en van Carolina Ivens beide in leven.
    Wij hebben ons van voormeld overlijden verzekerd. Waarvan akte, door ons ten Gemeentehuize in dubbel opgemaakt, en na voorlezing geteekend met de Komparanten.
