    Henricus Nys, werkman, oud dertig jaren, geboren en woonachtig te Bornhem, welke ons vertoond heeft een kind van het mannelijk geslacht, eergisteren om negen ure des avonds geboren in deze gemeente, Puersche Steenweg, van hem verklaarder en van zijne echtgenoote Maria Christina vermeieren, huishoudster, oud zeven en twintig jaren, hier geboren en woonachtig, en aan welk kind hij verklaard heeft te willen geven de voornamen van Carolus Ludovicus.
    De voormelde vertooning en verklaring gedaan ter tegenwoordigheid van Ludovicus Josephus Brys, hulponderwijzer, oud dertig jaren en Camillus Spiessens, schoenmaker, oud vier en dertig jaren, beiden woonachtig in deze gemeente.
    Waarvan akte, in dubbel, door ons ten Gemeentehuize opgemaakt, en na voorlezing geteekend door ons met den vader en de getuigen.

    Joannes Vermeiren, daglooner, oud zestig jaren en Carolus Ludovicus Vermeiren, fabriekwerker, oud een en twintig jaren, beiden woonachtig in deze gemeente, de eerstgenoemde grootvader van den overledene en de tweede oom, welke ons hebben verklaard dat gisteren om tien ure des avonds in zijne ouders woning, Puersche Steenweg, alhier overleden is Carolus Ludovicus Nys, geboren te Bornhem, den een en dertigsten october dezes jaars en daar woonachtig, zoon van Henricus Nys, werkman, oud dertig jaren en van Maria Christina Vermeiren, huishoudster, oud zeven en twintig jaren, gehuisschen woonachtig in deze gemeente.
    Wij hebben ons van voormeld overlijden verzekerd.Waarvan akte, in dubbel, door ons ten Gemeentehuize opgemaakt en na voorlezing geteekend door ons en de tweede verklaarder, de eerste heeft verklaard niet te kunnen schrijven bij gebrek van geleerdheid.
