    Ten jare duizend negen honderd en twaalf, den acht en twintigsten October te tien ure voormiddag, verscheen voor ons Theodoor De Maeyer, schepen, afgevaardigde Ambtenaar van den Burgerlijken stand der gemeente Bornhem, arrondissement Mechelen, provincie Antwerpen.
    Alexander Blommaert, metser, oud drij en twintig jaren, geboren en woonachtig te Bornhem, welke ons vertoond heeft een kind van het mannelijk geslacht, heden om twee uren des nachts, geboren in deze gemeente, Puersche Steenweg, van hem verklaarder en van zijne echtgenoote Philomena De Clercq, huishoudster, oud vijf en twintig jaren, hier geboren en woonachtig en aan welk kind hij verklaard heeft te willen geven de voornaam van Ludovicus.
    De voormelde vertooning en verklaring gedaan ter tegenwoordigheid van Camillus Spiessens, schoenmaker, oud zeven en dertig jaren en Joannes Verdickt, brievendrager, oud zes en veertig jaren, beiden woonachtig in deze gemeente.
    Waarvan akte, in dubbel, door ons ten Gemeentehuize opgemaakt, en na voorlezing door ons geteekend met de getuigen, de vader heeft verklaard niet te kunnen schrijven bij gebrek van geleerdheid.
