    Victor De bruyn landbouwer, oud zeven en veertig jaren geboren en woonachtig te Bornhem, welken ons vertoond heeft een kind van het mannelijk geslacht eergisteren om vijf uren des avonds geboren in deze gemeente van hem verklaarder en van zijne echtgenote Catharina Van den bergh oud zeven en dertig jaren hier geboren en aan welk kind hij verklaard heeft te willen geven de voornamen van Joannes Franciscus.
    De voormelde vertooning en verklaring gedaan ter tegenwoordigheid van Fredericus Goedgezelschap horlogemaker oud zestig jaren en Alexander Vinck dagloner oud een en dertig jaren beide woonachtig in deze gemeente.

    hebben ingevolge artikel tachtig van het burgerlijk wetboek het volgende uittreksel ingeschreven: N° 407- L'an mil neuf cent quatre, le vingt-neuf septembre à deux heures du soir après constatution, nous Léon Barbier Echivin Officier de l'Etat-Civil de la ville de Mons, Province de Hainaut, chevalier de l'ordre de Léopold dressons l'acte de décès de Jean François De Bruyn, âge de vingt deux ans, soldat au deuxieme régiment de chasseurs à cheval, né à Bornhem y domicilie décédé hier à six heures du soir rue André Masquelier, Célibataire, fils de Victor De Bruyn et de Jeanne Cathérine Vandenbergh, sur la déclaration faite par Roméo Ledent agé de vingt quarte ans et Ernest Grandfils, agé de trente neuf ans respectivement sergent et Caporal an Bataillon d'administration,voisins du défunt,domiciliér à Mons, duquel acte il leur a été donné lecture et que nous avons signé avec les déclarents (signé) Ledent, Grandfils et Barbier.
