    Ten jare duizend acht honderd negen en zestig, den zeven en twintigsten december ten drij ure namiddag, verscheen voor ons Eduardus Van Roie, schepen, gedelegeerden Ambtenaar van den Burgerlijken Stand er gemeente Bornhem, provincie Antwerpen,
    Alphonsus Pauwels, wever, oud zes en dertig jaren, geboren en woonachtig te Bornhem, welken ons vertoond heeft een kind van het mannelijk geslacht op heden om een uer namiddag, geboren in deze gemeente, van hem verklaerder en van zijne echtgenote Rosalia Coeck, oud vijf en dertig jaren, hier geboren en aen welk kind hij verklaerd heeft te willen geven den voornaem van Judocus.
    De voormelde vertooning en verklaring gedaan ter tegenwoordigheid van Ludovicus Josephus Van Breedam, secretaris, oud negen en dertig jaren en Joannes Franciscus Segers, veldwachter, oud zeven en dertig jaren, beide woonactig in deze gemeente.
    Waarvan akte, door ons ten Gemeentehuize in dubbel opgemaakt, en na voorlezing geteekend met de getuigen, de vader heeft verklaerd niet te kunnen schrijven bij gebrek van geleerdheid, na gedane voorlezing.
