    In het jaar negentien honderd twaalf, den eersten dag der maand April verscheen voor ons, Ambtenaar van den Burgerlijken Stand der Gemeente, Maartensdijk Jan Stomphorst oud zes en twintig jaren, van beroep metselaar, wonende te Maartensdijk. Dewelke ons verklaarde, dat op den dertigsten dag der maand Maart dezes jaars, des nachts ten halftwaalf ure, binnen deze Gemeente ten zijnen huize (Wijk A numero honderd vier en vijftig C) is geboren een kind van het mannelijk geslacht uit hem comparant en zijne huisvrouw Willempje Pater, zonder beroep, wonende alhier aan welk kind gegeven wordt de voornaam van Hermanus.
    Zijnde deze verklaring geschied in tegenwoordigheid van Klaas Koornwinder, oud drie en vijftig jaren, van gebroep gemeentebode, wonende te Maartensdijk en Jacobus Fenijn, oud twee en vijftig jaren, van beroep veldwachter, wonende te Maartensdijk als getuigen.
    Hebbende wij de tegenwoordige Acte, na voorlezing, ondertekend met den comparanten de getuigen.
