Jacob van der Vorm (1905-1992) is geboren op 12 juli 1905 te Rotterdam en overleden 13 mei 1992. Hij was tuinder van beroep op de hoeve "Heuvelstein", die hij kocht in november 1929. De huidige boerderij is gebouwd in 1906, maar half 17e eeuwstond er al een boerderij met dezelfde naam op deze plaats. Zoals de naam suggereert is deze boerderij gebouwd op een heuvel (ong. 1 meter hoog) ter bescherming tegen overstromingen. In 1987 verkocht hij “Heuvelstein” met bijbehorend land aan zijnzoon Pieter van der Vorm.

Jacob was van beroep tuinder op de hoeve " Heuvelstein " , die hij had gekocht in 1929 .De huidige boerderij is gebouwd in 1906 , maar half 17 e eeuw stond er al een boerderij met dezelfde naam op deze plaats . Zoals de naam suggereerd is deze boerderij gebouwd op een heuvel ter bescherming tegen overstromingen . In 1987 verkocht hij " Heuvelstein " met de bijbehorende land aan zijn zoon Pieter van der Vorm .

Heuvelsteyn voor oorlogJacob van der Vorm was afkomstig uit Charlois, waar zijn vader Pieter van der Vorm tuinder was. In 1930 trouwde hij met Cornelia Maria van der Meer (1903-1986). Na hun huwelijk gingen ze op Heuvelsteyn wonen. Tegen het einde van de tweede wereldoorlog in maart 1944 werd door de Duitsers de polder onder water gezet. De hele familie moest evacueren en verhuisde naar de bakkerij van de schoonfamilie van der Meer aan de Charloisse Lagedijk 509 in Rotterdam. Alles rondom deboerderij stond onder water, maar de boerderij zelf bleef boven water, zodat de heuvel z'n waarde bewezen heeft. Bij een harde storm in november 1944 stortte wel, mede als gevolg van de door het water verzwakte fundering, de schuur in.

Bij de razzia van Rotterdam op 10 november 1944 werd de toen in Charlois verblijvende Jacob van der Vorm opgepakt door de Duitsers en in Duitsland te werk gesteld. Hij wist daar na enkele maanden te ontsnappen met Jan Vugt en Janus Verschoor en op 2februari 1945 keerde hij weer terug in Nederland. Al snel na de bevrijding op 5 mei 1945 was de polder alweer droog gemalen en keerde de familie terug naar Heuvelsteyn. Er werd direct begonnen met de herbouw van de schuur, zij het wel een stuk kleiner dan de oorspronkelijk schuur. Zowel woonhuis als schuur waren in de nieuwe situatie 10 bij 10 meter.

De oorspronkelijke arbeiderswoning is jarenlang bewoond geweest door zijn dochter Heiltje en haar man. Het koetshuis werd in 1963 verbouwd tot woning en daar ging zoon Pieter wonen. In 1986 verkocht Jacob van der Vorm deze boerderij met bijbehorendland aan zijn zoon Piet en verhuisde in december 1986 naar bejaardentehuis "De Klepperwei" in Rhoon.
