  • He was born on September 11, 1921 in Naarden, Noord-Holland, Nederland.

  • He died on October 3, 1993 in Houten, Utrecht, Nederland, he was 72 years old.
  • A child of Henricus (Henk) Uijtewaal and Adriana Allegonda van Oostrom
