Zijne Excellentie den Gouverneur Generaal van Nederlands Oost Indie,

Geeft met den diepsten eerbied te kennen: Holtz, Johan Luis, onderluitenant der Infanterie alg. stamboek No 60871 in garnizoen te Buitenzorg, dat adressant blijkens het hierbij overgelegd uittreksel uit het register tot inschrijving
der acten van geboorten gehouden te Ngandjoek in het jaar 1899, vader is van het kind van de vrouwelijke kunne, aan welk kind is gegeven den voornaam Emma, dat hij in het belang van dat kind in het huwelijk had willen treden met de
moeder van het kind, de inlandsche vrouw Rantijem, welk huwelijk echter niet had kunnen plaats hebben, vermits blijkens nevensgaande verklaring gepasseerd voor het hoofd van plaatselijk bestuur te Poerworedjo de moeder van het kind
intussen is komen te overlijden, dat tengevolge van dat overlijden er voor adressant niets anders op overblijft, dan een beroep te doen op de edelmoedigheid van Uwe Excellentie ter verkrijging van brieven van wettiging, redenen waarom hij zich met den diepsten eerbied tot Uwe Excellentie wendt met het eerbiedig verzoek dat het Haar moge behagen, hem wel brieven van wettiging te verlenen, ten behoeve van zijn bovenvermeld natuurlijk erkend kind.

Overlijdingsverklaring Rantijem, 19 Juni 1914

De ondergetekende verklaart dat in de desa Kepek (Boeloeredjo) district en afdeling Poerworedjo, op den 21e Februari 1913 is overleden de inlandsche vrouw Ratijem, moeder van het kind genaamd Emma, dat blijkans het uittreksel No 12 uit het register der acten van geboorten gehouden te Ngandjoek in het jaar 1903 door Johan Louis Holtz, destijds sergeant-majoor bij het Nederlandsch-Indische Leger, voor het zijne is erkend.

De Gouverneur-Generaal van Nederlandsch-Indie heeft goedgevonden en verstaan,

Het verzoek in te willigen en mitsdien genoemd kind, hetwelk op den 7e Juli 1898 te Poerwoasri (Kediri) geboren en onder de voornaam Emma in de registers van de burgelijke stand te Ngandjoek als naturlijk kind van Johan Louis Holtz ingeschreven en voorts bij authentieke akte van den 17e October 1903 No 12 voor den buitengewonen ambtenaar van de burgelijke stand aldaardoor den verzoek als het zijne erkend is, te willigen en daartoe te verleenen deze brieven van wettiging, met dien gevolge, dat genoemd kind in allen opzichten en van stonden aan zal worden erkend en beschouwd als een wettige dochter van Johan Louis Holtz.

Gegeven te Batavia, den zesden Augustus 1914, de Gouverneur-Generaal van Nederlandsch-Indie Idenburg.
