Het jaar achttienhonderd zes en tachtig den een en twintigsten januari om drie ure namiddag voor ons Leo Antoon Naudtz burgemeester ambtenaar van den burgerstand der gemeente Sint Gillis waas, provintie Oostvlaanderen, is gekomen Lodewijk Van Duyse, oud vijf en veertig jaren werkman, hier geboren en wonende den welken ons heeft vertoond een kind van het mannelijk geslacht, hier geboren gisteren om zeven ure des avonds in zijne woning Molenstraat nummer vijf van hem verklaarder en van Delphina Megroedt, oud vier en dertig jaren kantwerkster, hier wonende en geboren te Meerdonk zijne huisvrouw en waaraan hij verklaarde te willen geven den voornaam van Jan de vertoning en verklaring gedaan in de tegenwoordigheid van Jan Van Duyse oud twee en veertig jaren werkman en August De Schrijver oud vijf en vijftig jaren, kleermeker beide hier wonende, getuigen hiertoe door den komparant aanzocht en hebben wij onmiddelijk dezen akt opgesteld en na voorlezing benevens den tweeden getuige onderteekend, den eersten en den vader verklarende niet te kunnen naamtekenen door ongeleerdheid.
