    Tot de zestiende eeuw was verloskunde een vrouwenzaak. De omstandigheden waaronder vrouwen hun kind kregen zijn te vergelijken met die in de huidige ontwikkelingslanden. Kinderen werden thuis, in onhygiënische omstandigheden geboren. Er was geen goed licht, geen water of zeep. Om het beddengoed te sparen lag de kraamvrouw niet in bed, maar op de grond in het stro. Pijnstillers bestonden nog niet; hooguit greep de aanstaande moeder naar de drank om de pijn te verminderen.

    Terwijl een gemiddelde bevalling tegenwoordig twaalf uur duurt, waren vrouwen vóór de twintigste eeuw ongeveer zestien uur bezig. Bovendien duurde één op de drie bevallingen in die tijd meer dan twintig uur, wat de kans op complicaties alleen maar groter maakte. Na tuberculose was kinderen krijgen de belangrijkste doodsoorzaak voor vrouwen. Voor 1800 eindigde 1,3 procent van de bevallingen met de dood van de moeder. Ter vergelijking: in 2007 was dat 0,005 procent.
