    Heden den 13den Januari negentien honderd en zestien aan boord van bovengenoemd stoomschip, voor anker liggende op de Theems bij Tilbury, zijn voor mij Adriaan Buskop, gezagvoerder van gemelde bodem, verschenen Hendrik Jan Bierman, oud één en veertig jaren, en Frederikus Johannes Hintertür, oud vier en dertig jaren, welke verklaard hebben dat Hendrik de Boer, kapitein van het Stoomschip "Maashaven", laatst gedomicileerd te Rotterdam op den dertienden Januari negentien honderd en zestien des namiddags ongeveer 5 uur is overleden (nadat hij 4 uren te voren bewusteloos aan boord was gehaald) op meergenoemden bodem in den ouderdom van 42 jaren, echtgenoot van Janneke Vil, wonende te Rotterdam.-
    Hiervan is deze verklaring opgemaakt die, na voorlezing, door de comparanten is mede ondertekend
