In het jaar negentienhonderd een, den drieentwintigsten der maand October verscheen voor mij Diderik Hendrik Jas Ambtenaar van den Burgerlijken Stand der Gemeente Dubbeldam Johanna Elizabeth van der Mooren van Koeverden, oud dertig jaren, vroedvrouw wonende te Dubbeldam welke mij verklaard heeft, dat op den tweeentwintigste dezer maand des na middags ten zeven ure, te Dubbeldam in het huis geteekend wijk A nummer negenenvijftig is bevallen van een kind van het mannelijk geslacht
Maria de Vogel, zonder beroep, echtgenoote van Jacob van Heeren, arbeider, beiden wonende alhier, aan welk wordt gegeven de voornaam van Pieter. Verklarende aangeefster bij de bevalling tegenwoordig te zijn geweest, terwijl de vader door afwezigheid is verhinderd, zelf de aangifte tedoen.
Deze verklaring is geschied in tegenwoordigheid van Roelof Anton Damaté, oud drieendertig jaren, ontvanger wonende te Dubbeldam en van Teunis Stehouwer, oud eenenveertig jaren, veldwachter wonende te Dubbeldam alsgetuigen.
Waarvan dadeijk deze Acte is opgemaakt, aan de aangever en de getuigen voorgelezen en door mij met hen onderteekend.
