In het jaar achttien honderd negen en tachtig , den derden der maand September , verscheen voor mij Ambtenaar van den Burgerlijken Stand der Gemeente Zwijndrecht ; Jan Broeksmit , oud acht en vijftig jaren , geneesheer , wonende te Zwijndrecht welke , mij verklaard heeft , dat op den tweeden dezer maand , des namiddags ten zeven ure , te Zwijndrecht in het huis geteekend wijk B nummer ---- ----- , is bevallen van een kind van het mannelijk geslacht , Maria Braber , zonder beroep , wonende alhier , echtgenoote van Pieter van HEEREN , timmerman mede alhier , aan welk kind wordt gegeven de voornaam van PIETER , verklarende deaangever bij de bevalling tegenwoordig gevend te zijn en dat de vader door afwezigheid verhinderd is zelf de aangifte te doen .
Deze verklaring is geschied in tegenwoordigheid van Johannes Jacobus Damate , oud vier en twintig jaren Ambtenaar ter secretarie , wonende te Zwijndrecht en van Cornelis Adrianus Eijkelenboom , oud drie en dertig jaren , veldwachter , wonende te Zwijndrecht als getuigen .
Waarvan door mij dadelijk deze Akte is opgemaakt , aan den aangever en de getuigen is voorgelezen , en door mij met hen onderteekend .
