    Hij is 20-01-1912 opgenomen en 20-10-1913 overleden en 24-10-1913 begraven in graf A 96. Hij was toen 74 jaar gehuwd en kwam van Dubbeldam.
    De heren Hoek en vd Veer hebben zijn kist gemaakt. Hij is gedragen door de heren Boon, Versteeg, Pettinga, vd Gallen, Hoek, de Veer en Rothert. De Grafdelver was dhr Bakx. Zijn patiënten was 424.
    Het graf is in 1953 geruimd
