The Wilmington, Delaware form was signed "Dougherty and McGillin" in the undertaker line, but a different undertaker was used in Philadelphia (This may indicate that the body was released to his cousins in Philadelphia, who were surnamed Dougherty and McGillin). The Philadelphia address used prior to burial was given as 2215 North 15th Street, Philadelphia (currently a residence in the neighborhood). This burial certificate states that he was buried in New Cathedral Cemetery, Philadelphia, Pennsylvania. The original burial part of the certificate gives 31 January 1911 as the burial date, but the other forms and an office stamp on the Delaware part of the form (perhaps from the City of Wilmington when it processed the form) give the year as 1912.
- Actual burial was in Old Cathedral Cemetery, Section U, Range 6, Lot 25, according to records provided by the Archdiocese of Philadelphia Cemeteries Office to Robert M. Simon on 17 September 2012. Date of burial was 31 January 1912. Tombstone on site is intact.
